Minimally Invasive Gynecologic Surgery (MIGS)
- A MIGS surgeon addresses complex varieties of fibroids, endometriosis, pelvic pain, ovarian cysts, adenomyosis, etc. Complex gynecologic conditions may require more skill and experience.
- Minimally invasive surgery can be offered for most gynecologic conditions.
- Minimally invasive surgery has better outcomes, less complications, and faster recovery.
- Ask your doctor about minimally invasive surgery if you are being considered for surgical intervention.
- Surgical modalities:
- Laparoscopy (tiny incisions of the abdomen) is the most common method of minimally invasive surgery.
- Robotic surgery is similar to laparoscopy and available depending on the patient’s clinical circumstances.
- Hysteroscopy is surgery inside the uterus with the help of a camera.
- Vaginal surgery can be performed for specific diagnoses and depending on the patient’s clinical circumstances.
- Laparotomy (large incision of the abdomen) is performed if minimally invasive surgery is not possible
Endometriosis & Pelvic Pain Program (EP3)
- The Endometriosis & Pelvic Pain Program is based in Bronx, NY.
- The Program is run by MIGS surgeons and led by Dr. Palvia (Lincoln/Harlem) and Dr. Barraez (Jacobi/North Central Bronx).
- Your gynecologist may refer you to the program if you have complex endometriosis or chronic pelvic pain.
- Causes
- Endometriosis is a chronic inflammatory condition that commonly causes chronic pelvic pain and infertility.
- Endometriosis occurs when tissue inside the uterus escapes from the fallopian tubes and implants within the abdominopelvic cavity.
- Non-gynecologic causes of chronic pelvic pain include problems with your bladder, intestines, pelvic floor muscles, and pelvic bones.
- Chronic pelvic pain can also be caused by fibroids, ovarian cysts, pelvic infections, pelvic trauma, and abdominopelvic surgeries.
- Management & Treatment
- You will undergo a thorough physical exam, diagnostic imaging, and possible surgery to better understand the severity of disease.
- The program works with other departments (Urology, Gastroenterology, Colorectal Surgery, Physical Therapy, Lifestyle Medicine, Mental Health) to provide comprehensive care.
- Endometriosis and chronic pelvic pain can be managed thru a combination of conservative, medical, and surgical therapies.
- The goal of therapy is to improve your quality of life.

- A Urogynecologist is an OB/GYN that specializes in treatment of pelvic organ prolapse and bladder or bowel leakage.
