What you can expect during your visit
- Your provider will ask about your general wellbeing, your health concerns, relationships, sexual activities, needs for preventive care such as vaccinations, the need for assistance with smoking cessation or substance use issues and symptoms of depression and anxiety.
- You can ask your provider about any other medical, psychological or social issues that concern you.
- You can designate a health care proxy, including a same-sex partner.
- Your services will be delivered with complete confidentiality.
- You are free to use the restroom that matches your gender identity or expression.

Is the information I share with my provider private?
Yes. Anything you share with us about your health is used to help you get the best care you need and will not be shared with others.
Does Elmhurst Hospital provide translation services?
Yes. Elmhurst provides free translation services for all languages and sign language interpreters.
When do I find out about my test or screening results?
Results will be available to you within MyChart, which is part of the electronic health record. Please make sure we always have your correct contact information in case we receive test results and need to reach you before your next scheduled appointment.
Can I get a copy of my medical record?
Yes. To transfer your records to us, call the Correspondence Supervisor at 718-334-3144 or visit us in room EB22, Monday – Friday 8 a.m. – 4 p.m. You can access your health record online via MyChart — a patient portal to your electronic health record.
May I still be a patient if I don’t have insurance?
Yes. We treat all patients even if they don’t have insurance. There are representatives in the clinic available to assist you with financial counseling and insurance enrollment for many plans, including Medicaid and Medicare. Some patients may qualify for NYC Care, a health care access program that guarantees low-cost and no-cost services offered by NYC Health + Hospitals to New Yorkers who do not qualify for or cannot afford health insurance based on federal guidelines.
