NYC Health + Hospitals’ Senior AVP Dr. Hannah Jackson named to Modern Healthcare’s “40 Under 40” List

Dr. Hannah Jackson is recognized for her work driving equitable access to care through data-driven solutions and team-based healthcare models

NYC Health + Hospitals’ Senior Assistant Vice President and Chief of Staff for the Office of Ambulatory Care and Population Health Dr. Hannah Jackson has been named to Modern Healthcare’s 2025 “40 Under 40” list in recognition of her significant work improving healthcare access and equity across New York City. Dr. Jackson has led substantial improvements to access to primary care at NYC Health + Hospitals, the largest public healthcare system in the country. Over the last year, Dr. Jackson has helped lead the growth of primary care to nearly 450,000 New Yorkers, extending care to more than 15,000 new patients compared to the year prior. With her focus on continuity between patients and their doctor, she has increased continuity to 78% in FY24, the highest rate of continuity since the public healthcare system started measuring it several years ago. She also led the redesign of our scheduling system to improve access, and reduced our primary care no-show rate by approximately 50% in the past year, with our lowest no-show rate ever of only 26%.

She is the architect of our clinical pharmacist program that has helped our patients to have the best chronic disease control our system has ever seen. Dr. Jackson is a stalwart believer that healthcare is a human right, and her enthusiasm for realizing that right through our health system’s world-class primary care is infectious.”

Dr. Jackson has made foundational changes in healthcare delivery that provide a blueprint for innovation and change for other healthcare systems. During the COVID-19 public health emergency, Dr. Jackson created a telehealth system that served hundreds of thousands of New Yorkers across more than one million visits. She also established the first clinical pharmacist program in our system, currently with over 40 lines for clinical pharmacists in our ambulatory care practices. This program has helped nearly 20,000 patients to date, with an overall A1c change of more than 2 points for diabetic patients. the highest control rate in the public health system’s history.

Also, as part of the scheduling system overhaul that she led, NYC Health + Hospitals’ primary care practices have seen 24% more new patients in 2024 than the same period of time last year. In terms of health outcomes for patients, chronic disease outcomes in 2024 were the best they have ever been. Thanks to Hannah’s work, NYC Health + Hospitals has currently achieved 69.1% A1c control for all diabetic patients and 79.6% blood pressure control for all hypertensive patients — the highest control rates in the public health system’s history.

Dr. Jackson earned her master’s degree in public health along with her medical degree at Harvard University. After finishing residency at Yale School of Medicine, Dr. Jackson was an assistant professor at Georgetown University School of Medicine before joining NYC Health + Hospitals in 2018.
