In celebration of National Skilled Nursing Care Week, formerly Nursing Home Week, NYC Health + Hospitals/Post-Acute Care proudly celebrates its five skilled nursing facilities and Long-Term Acute Care Hospital, recognizing their new services and high-performance ratings. In the past year, NYC Health + Hospitals/McKinney and NYC Health + Hospitals/Sea View began offering Parkinson’s care, NYC Health + Hospitals/Carter launched telemetry services and hospice care, NYC Health + Hospitals/Gouverneur launched enhanced dining for its residents, and NYC Health + Hospitals/Coler opened The Coler Café.
The facilities are widely recognized for providing high quality care. In 2024, Newsweek ranked Carter, Coler, Gouverneur, and Sea View in the Top 10 of Best Nursing Homes 2024 in New York State, and McKinney ranked in the Top 30. U.S. News & World Report recognized NYC Health + Hospitals/Carter, Gouverneur, McKinney, and Sea View for high performance in short-term rehabilitation, for the latter three sites, high performance in long-term care. NYC Health + Hospitals/Sea View, Coler, Carter, Gouverneur and McKinney consistently earn four- and five-star ratings from the U.S. Centers for Medicare & Medicaid Services (CMS), which designates five-star rated skilled nursing facilities among the top 10 percent of nursing homes in the country. Additionally, NYC Health + Hospitals/Post-Acute Care facilities are ranked in the top 1st quintile in the New York State Department of Health Nursing Home Quality Pool.

The following premiere clinical services are available through the public health system’s long-term care facilities:
Memory Care Unit – NYC Health + Hospitals/Coler offers award-winning memory care services for New Yorkers living with Alzheimer’s and dementia. Coler residents receive daily engagement to promote socialization opportunities through art classes, gardening, event gatherings, and the Music & Memory Program in a safe, structured environment of community and support which nurtures positive relationships and person-centered care. Around-the-clock coordination and integration of clinical care and ancillary services are provided by teams of physicians, nurses, and a variety of healthcare specialists, including dietitians, rehabilitation therapists, social workers, and recreation therapists.
“Coler proudly serves as the largest safety-net post-acute care facility in our public health system, providing long-term care to residents with a wide-spectrum of medical and behavioral health needs,” said NYC Health + Hospitals/Coler Chief Executive Officer Stephen Catullo, LNHA, MBA. “This Skilled Nursing Care Week, we honor our nationally and internationally recognized Memory Care Units who exemplify our commitment to providing evidenced-based care which optimizes resident outcomes and enhances their quality of life.”
Rehabilitation Therapy – NYC Health + Hospitals/Gouverneur offers state-of-the-art rehabilitation in a 4,000- square foot space. An Academic Affiliate of the NYU Rusk Institute, Gouverneur has a team of physical and occupational therapists and speech pathologists who leverage advanced techniques to help individuals recover from an injury, illness, or surgery in a friendly, welcoming environment.

Parkinson’s Care – NYC Health + Hospitals/McKinney receives new certification in Parkinson’s Care.Each year, 90,000 people in the United States are newly diagnosed with Parkinson’s Disease, a neurological condition affecting the brain. There’s no cure for the disease, but care and treatments can help relieve symptoms and maintain quality of life. As part of the Parkinson’s Foundation (PF) Community Partners in Parkinson’s Care, McKinney staff receive training and education to support and provide treatment to meet the unique needs of people living with Parkinson’s Disease. McKinney teams tailor individualized care plans with a combination of medication and supportive therapies like physical therapy, occupational therapy, speech therapy as well as nutrition.

Telemetry Expansion – NYC Health + Hospitals/Carter recently opened Telemetry and Cardiac Rehabilitation services to nursing home residents, using an advanced remote technology for monitoring while participating in skilled therapy. Telemetry is designed for individuals who are admitted for skilled rehabilitation after an acute cardiac event, cardiac surgeries, other cardiac procedures, and/or exacerbation of congestive heart failure. It reduces and prevents cardiac complications which can lead to re-hospitalization.
In addition, the Carter has begun offering hospice care in partnership with Hospice of New York. Care and clinical services are provided by Carter staff, and Hospice of New York team members make regular visits, including hospice nurses who offer their expertise in pain and symptom management. Additionally, hospice bereavement counselors are available for the family, other residents and the Carter staff.

Traumatic Brain Injury – NYC Health + Hospitals/Sea View opened the first Long Term Care Traumatic Brain Injury Program in New York City. Sea View offers a 21-bed unit designed to accommodate patients in the active restorative rehabilitation program; and a dedicated 38-bed extended care unit for brain injured patients. Sea View aims to restore motor and cognitive functions, maximize quality of life, preserve human dignity and ultimately discharge the patient to a lower level of care or the community.
