Notice of Nondiscrimination for Patient-Related Services

- Provides free reasonable modifications, including auxiliary aids and services, to people with disabilities to communicate effectively, such as:

- Provides free language assistance services to people whose primary language is not English, such as:
- Qualified interpreters
- Information written in other languages, both electronically and in hard copy

If you believe that NYC Health + Hospitals failed to provide these services or discriminated in another way on the basis of race, color, national origin (including limited English proficiency and primary language), age, disability, gender identity or sex (includes but not limited to sex characteristics, including intersex traits, sexual orientation, sex stereotypes, and pregnancy or related conditions), you can file a grievance with the Section 1557 Coordinator at the facility where the incident occurred.
You can file a grievance by email or phone. If you need help filing a grievance, the Section 1557 Coordinator is available to help you. Please see the list above for contact information at each facility.
