The NYC IPA for Special Populations is soliciting proposals from organizations seeking to join the NYC IPA Care Management Network. Qualified organizations interested in delivering care management services for people experiencing homelessness in New York City are encouraged to apply.

NYC Health + Hospitals has incorporated a new subsidiary independent practice association, the NYC IPA, in partnership with the City of New York and New York State Department of Health to be a vehicle for advancing health equity by reorienting the health care system to address social determinants of health for New Yorkers experiencing homelessness.
About the NYC IPA
By coordinating and investing in the systems and people who care for patients experiencing homelessness, the NYC IPA will strengthen longitudinal care relationships, improve patients’ health and social care outcomes, and increase the value delivered through healthcare settings. Organizations that are under contract with the NYC IPA will receive and dedicate resources to provide tailored care management services to Program Members, including Engagement and Navigation, Core Care Management, and Social Services.
The program is funded by a directed payment approved by the New York State Department of Health (NYS DOH) and Centers for Medicare and Medicaid Services (CMS). NYS DOH and CMS require New York City region Medicaid Managed Care Organizations (MCOs) to participate in the population-based payment model, whereby the IPA is responsible for the total cost of care for single adults experiencing
Commented [BI17]: Visual to be updated to align with expectations homelessness who have had at least 7 days in shelter in the past 12 months, who are enrolled in Medicaid Managed Care (MMC) or Health and Recovery Plan (HARP), and are NOT enrolled in Health Home. This investment in care management will transform NYC IPA Program Members’ engagement and experience with medical care, behavioral health services, and social service supports to achieve stable, improved health and build stronger connections to longitudinal care.
IPA Program Members often have complex physical health, behavioral health, and social needs. IPA Program Members may lack durable connections to outpatient longitudinal care and may either rely on inpatient and emergency department services to address worsening health concerns or avoid healthcare or social service providers altogether. The NYC IPA network approach enables a higher level of collaboration between social services providers and the health system. Participating organizations are expected to take a whole-person approach to care delivery, invest in care management, and collaborate across health systems in New York City to improve the value and coordination of care provided to IPA Program Members.
