Independent Practice Association

NYC Health + Hospitals has launched a new subsidiary independent practice association (NYC IPA) to serve special populations across New York City starting in April 2024. The New York Community IPA (NYC IPA) will provide care managementand social services to New Yorkers covered by Medicaid (Medicaid Managed Care or Health and Recovery Plan) who have spent at least 7 days in a DHS single adult homeless shelter over the past year. Organizations that are awarded a contract to participate in the NYC IPA network will provide non-clinical services that further the IPA’s mission.  

To indicate your organization’s interest in providing eligible services under contract with the NYC IPA, please submit the required materials to the NYC IPA at NYCIPA@nychhc.org by [date]. The NYC IPA will process requests on a rolling basis as they are submitted until the need for care management services is met. Interested organizations are expected to provide documentation validating that they meet the NYC IPA’s minimum organizational qualifications to provide services, the service area(s) that the organization is interested in providing for the NYC IPA, and contact information for key stakeholders at the organization.

NYC IPA Network

  • IPA Healthcare Participants: Healthcare provider organizations and federally qualified health centers (FQHCs) that currently receive managed care attribution for NYC IPA-enrolled patients and meet the NYC IPA’s minimum qualifications are potentially eligible for contracting as a Participant.   • IPA Care Management Partners: Care management agencies and community-based organizations that are interested in providing services under contract with the IPA are potentially eligible for contracting for at least one or more care management intervention as a Care Management Partner.   • Organizations interested in joining the NYC IPA Network must meet the minimum eligibility requirements and submit the following materials to be considered. The NYC IPA will process requests on a rolling basis as they are submitted until the need for care management services is met.   • IPA Participant Network Agreement: The template agreement for IPA Healthcare Participants and IPA Care Management Partners. This document details the contractual expectations for all organizations in the IPA network.
  • IPA Scorecard, Audit Template, Reporting, and Budgeting Templates: IPA Network members will be responsible for reporting on a monthly basis in alignment with the expectations set forth in the IPA Participant AgreementIPA Participant Network Agreement.
