5 Things To Know about Our Short-Term Rehab and Skilled Nursing Facilities

5 Things To Know about Our Short-Term Rehab and Skilled Nursing Facilities

Patients in need of rehabilitative or skilled nursing care after a hospital stay don’t have to look beyond our health system to find the best post-acute care services in NYC.

That’s because NYC Health + Hospitals has five skilled nursing facilities that all offer short-term rehabilitation and specialized care for medically complex conditions. “These facilities are an extension of our hospitals for those who need that extra support to feel their best,” says Maureen McClusky, Senior Vice President, PostAcute Care, NYC Health + Hospitals. “The quality of care in these facilities is top notch. I often say that our post-acute care is our ‘best kept secret.'” Here’s what you need to know to get in on the secret:

  • Our skilled nursing facilities are among the best in the country.
From lower Manhattan to Roosevelt Island, the majority of our facilities have five-star ratings from the Centers for Medicare and Medicaid Services (CMS). These highest possible ratings put our facilities in the top 10% of all skilled nursing facilities in the country. NYC Health + Hospitals/Gouverneur and NYC Health + Hospitals/ Sea View have also been named the best in the industry in the U.S. News & World Report’s “Best Nursing Homes” report for 2016-2017.
  • We have NYC’s only certified facility that provides specialized care for high acuity and medically fragile patients.
NYC Health + Hospitals/Carter is also home to a certified 201-bed Long-Term Acute Care Hospital (LTACH), a facility that is designed and equipped to provide 24/7 specialized care for medically complex patients. The modern facility is staffed by physicians, nurses, respiratory care practitioners, and specialists in many areas, including pulmonary medicine and cardiology. The LTACH’s ventilator unit is renowned for successfully weaning patients off ventilators.
  • We’re a pioneer in traumatic brain injury care.
NYC Health + Hospitals/Sea View was the first facility in downstate New York to establish a state-certified Traumatic Brain Injury Center. The Center has a 21-bed unit for patients who need active restorative rehabilitation and a 38-bed unit for braininjured patients who require extended care. It is staffed by an interdisciplinary team of expert providers and rehab specialists.
  • Looking for home care services? We have that, too.
Our system also has its own certified home health agency. NYC Health + Hospitals/At Home provides high quality, personalized home care services for individuals who need special support after a hospital stay or to avoid hospitalization. Patients can get help with managing a chronic care condition and medications or recovery after surgery in the comfort of their own homes. The agency serves patients living in Manhattan, Brooklyn, Queens, and the Bronx. To refer a patient to our home care services, call 1-866-692-HOME (4663).
  • Transferring from our hospitals to post-acute care facilities has never been easier.
A newly established centralized admission team is now in place to ensure a smooth and swift referral process for those patients seeking admission into one of our skilled nursing facilities or Carter’s LTACH.

“When patients receive all their services within NYC Health + Hospitals, everyone benefits. Patients receive the best possible care, because all aspects of their experience are coordinated and aligned,” McClusky says.
